Many insurance companies reimburse medical mileage at the IRS standard business mileage rate. Check your policy or ask your insurance company which rate applies to your claim.
If you submit a valid medical mileage claim to your insurance company and are not reimbursed within 30 days, you may also be entitled to interest.

an auto accident. At the end of March, 2025 Luis is ready to submit a medical mileage reimbursement request.
With 4 total physical therapy appointments in March, Luis drove a total round trip of 18 × 4
miles (72 miles).
Luis would be owed: 72 × 0.70 = $50.40
2025 Other Mileage Categories
21 cents per mile driven for medical or moving purposes for qualified active-duty members of
the Armed Forces.
14 cents per mile driven in service of charitable organizations.
