Many insurance companies reimburse medical mileage at the IRS standard business mileage rate. Check your policy or ask your insurance company which rate applies to your claim.
If you submit a valid medical mileage claim to your insurance company and are not reimbursed within 30 days, you may also be entitled to interest.
2026 Medical Mileage Example
In February, 2026 Priya drives 26 miles round trip each week to physical therapy appointments related to
an auto accident. At the end of February, 2026 Priya is ready to submit a medical mileage reimbursement request.
With 4 total physical therapy appointments in February, Priya drove a total round trip of 26 × 4
miles (104 miles).
Priya would be owed: 104 × 0.725 = $75.40
In August, 2026 Tom drives 15 miles round trip each week to physical therapy appointments related to
an auto accident. At the end of August, 2026 Tom is ready to submit a medical mileage reimbursement request.
With 4 total physical therapy appointments in August, Tom drove a total round trip of 15 × 4
miles (60 miles).
Tom would be owed: 60 × 0.76 = $45.60
